Thank you for your interest in receiving the COVID-19 vaccine at Jackson Health System. There is no charge for this vaccine at Jackson.
Currently, Jackson is exclusively providing the COVID_19 vaccine manufactured by Pfizer, and only for children ages 5 to 11.
Vaccination is widely available for people age 12 and older at retail pharmacies, grocery stores, and other locations.
Children age 5 to 11 who are vaccinated at Jackson will automatically be registered for a follow-up appointment to receive the second dose at the same location 21 days after the first.
The vaccine is only fully effective after receiving both shots, and we require your commitment to receive both at Jackson.
Both doses of the vaccine must be administered by Jackson. We are unable to provide a second dose for children who received their first dose from another provider.
When we schedule your appointment, it will only be for the person registered. We are unable to vaccinate family members, others who live in your household, or other people during your appointment.
Is this appointment for a patient who is between the ages of 5 and 11?
Will the child's parent or legal guardian be present at the appointment to provide consent?
Can you bring an official document with the child's date of birth, such as a birth certificate or passport?
Do you live - either fulltime or parttime - in Florida?
Patients under 18 must have a parent or legal guardian present to provide consent.
Jackson requires proof of age for all patients at time of vaccination.
Jackson is no longer providing COVID vaccine for patients over 11. Vaccination is widely available for other patients at retail pharmacies, grocery stores, and other locations.

The COVID-19 vaccine has been found to be safe and effective for the vast majority of people.
Because the vaccine is so new, however, we recommend discussing vaccination with your doctor and then coming back to this link.
The State of Florida is only permitting Jackson to vaccinate Florida residents at this time.
The State of Florida currently requires these documents to prove your residency. We cannot provide an appointment for you at this time.
